Summary
The Case Manager provides social services support to the patients enrolled in the Ryan White Program. Case management includes assisting patients with obtaining resources needed to promote healthy living with HIV disease.
Knowledge, Skills, and Abilities
- Basic knowledge about HIV/AIDS
- Basic knowledge about HIV/AIDS medications
- Basic knowledge about rapid HIV antibody testing
- Knowledge of community resources.
- Ability to communicate effectively with people of all educational, social cultural and economic backgrounds.
- Be able to travel between clinic sites.
- Qualifications/Education Requirements
Graduation from an accredited school of social work or a related field is required. A Master’s level degree is preferred but a Bachelor’s degree with experience in Case Management is acceptable. Should hold a license in their respective field or be licensable. The Case Worker should have the ability to work independently and as a member of an inter-disciplinary team.
Core Competencies
- Provide client advocacy services to connect clients to community services.
- Assess patient economic status to determine eligibility for emergency assistance programs or for state and federal assistance programs.
- Conduct outreach/prevention activities to include target audiences for minorities, women, adolescents, drug users and persons released from incarceration, including prevention for positives
- CAREWare Data Entry
- Prevention, Risk Assessment and Risk Reduction counseling.
- Conduct rapid HIV tests or refer to appropriate medical services for testing or follow-up on positive tests.
- Transportation assistance
- Treatment/adherence counseling
- Rx assistance (including ADAP applications)
- Referrals for legal services, permanency planning
- Program orientation with completion of a Psychosocial Evaluation and Care Plan
- Provides Health Education on HIV treatment and related topics
- Helps organize the Consumer Advisory Board (CAB)
- Attends all Ryan White Clinics
- Attends state Care and Services meetings as proxy for Program Director when designated to attend
