Community Health Centers are local, non-profit health care centers providing comprehensive primary care to all residents regardless of ones ability to pay for such services. We serve as a patients safety net and maintain an open-door policy, providing high quality affordable care for both self pay and patients with insurance coverage. Also known as Federally Qualified Health Centers, the centers are governed by a local board of directors and are generally located in areas where care is needed but in short supply.

You may visit any of our providers by calling to make an appointment at 877.374.4991 or fill out a request for appointment on our website. We will be happy to assist you with transferring your records to one of our Providers when you make your first appointment, or your clinic’s Patient Service Representative will help you at the time of your first visit.

We have providers on-call for established patients. 

No, services at Coastal Family Health Center are not free and payment is due at the time services are rendered. Payment plans are available. We provide sliding fee discounts to eligible patients based on federal poverty guidelines. Click here to view a list of charges.

It is our policy that all payments, co-payments, co-insurances, and deductibles are due at the time of service. Statements are mailed out on a periodic basis for any outstanding balances owed. Please ask our cashier for further assistance on payment arrangements.
Yes, we accept all private/commercial insurance plans to include Medicare and Medicaid.

Proof of identification/address: Driver’s license, birth certificate, employment ID, or social security card

Proof of Income: Prior year tax return,recent pay stubs, pensions, social security benefits, disability, veteran’s benefits, unemployment compensation, retirement, child support or alimony payments, letter with balance on Electronic Benefit Transfer (EBT) card, or reference letter to verify unemployment status

If income is paid in cash, please provide a letter from employer to include the current date, date hired, employee’s name and address, employer’s name/address/phone number, wages per hour and how many hours of work per week, and frequency of pay period. The letter must be signed and dated by the employee and employer.

For patients with insurance:

Insurance: Health and/or dental insurance, prescription coverage, or supplement insurance card

Medicaid: Medicaid card

Medicare: Medicare card

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing prior to  your medical service or item when requested. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or picture of your Good Faith Estimate.
