This notice is to inform you about an information security situation that may have affected some of our patients. We understand this may be concerning, and we want to provide you with as much information as possible.

On April 16, 2025, we discovered an electronic disclosure of protected health information was made to one of Coastal Family Health Center’s funders. The information included names, dates of service, medical provider and visit type.

Due to the number of potentially affected individuals, we are providing this notice as a substitute for individual notifications. This notice is in compliance with applicable data breach notification laws.

We assure you that we are committed to safeguarding your sensitive personal information and have taken steps to fortify the protective measures which are already in place.  Upon notification, Coastal Family Health Center immediately contacted the funder and asked that protected health information be destroyed. Since the incident, we verified the information was electronically deleted, provided additional training to staff involved, and implemented additional precautions to reduce the risk of future unauthorized disclosures.

We take very seriously our role in safeguarding your personal information and using it in an appropriate manner.  Coastal Family Health Center apologizes for the stress and inconvenience this situation has caused you.
